  To establish a new service:

1.   Fill and sign the Master Service Agreement*
2.  Fill and sign Long Distance form if you choose to have USCom LD services
3.  Fax it to 800-795-6724

  To switch an existing service:

1.   Fill and sign the Master Service Agreement
2.  Fill and sign Long Distance form if you choose to have USCom LD services
3.  Fax it to 800-795-6724 with a copy of your current phone bill

1.  Fill and sign Long Distance form
2.  Fill and sign Responsible Organization form if you also would like an 800 number/numbers.
